Equipment Financing with Band Pro

Credit Application
If you wish to apply for NET30 payment terms with Band Pro Film & Digital, Inc., please complete and submit our Credit Application for review. Submission of the form does not guarantee an extension of credit to yourself or your organization. Also, please note that for all new customers your first purchase must be COD before a line of credit can be established.

Credit Card Authorization
If you choose to pay for your purchase using a personal or business credit card, complete our Credit Card Authorization form and submit it for approval. Please double check that you have included a photo copy of your credit card (front and back) and your drivers license. Also, please make sure that all information on the copies is legible. Section 6378 Exemption Certificate / Post Production
If your company's business meets the criteria established by California Regulation 1532 regarding post production services, you may be eligible for a partial exemption from sales and use tax. VERY IMPORTANT: It is the responisiblity of the company completing and submitting this form to verify that they are eligible for this exemption. Band Pro is not authorized or empowered to either confirm or deny your company's eligibility. Please consult with your accountant before submitting this form.
